In North America:
PrePaid Visas (gift cards) are available from many retailers and banks. The setup cost can vary dependant on the retailer (usually 3.95 to 9.95). Gift cards are usually not reloadable. A monthly fee or usage fee may apply.
Reloadable cards follow the same setup fees, but can be reused over and over. With these, you can control the usage. Find reloadable Visas at Wal Mart, Target, or go to NetSpend.com to find other retailers.
Use My Wallet and eWallet Express are 3rd party services that accept deposits through Visa, Mastercard, and bank transfers. The balance is then available (minus a fee) for depositing into gaming accounts. Withdrawals can be made into these Wallets, and can be received by check or directly deposited into a bank account, also with a withdrawal fee.
